DS-82 Form 2024 - 2025

Form DS-82, US Passport Renewal Application for Eligible Individuals is the Department of State form that’s used for requesting a passport renewal. Although not many of us choose to renew our passports unless it’s not used much or at all, you can use the DS-82 to request a passport renewal. 

You’re not required to file Form DS-82, but you can use it instead of DS-11. To fill out Form DS-82, you must answer yes to the following questions.

Form DS-82 Questions

  • You can submit your most recent US passport with DS-82.
  • You were at least 16 years of age when you received your most recent US passport.
  • Your passport was issued to you less than 15 years ago.
  • The US passport you’re renewing isn’t damaged, lost, stolen, or mutilated. 
  • Your most recent passport wasn’t in non-compliance with 22 C.F.R. 51.41 or limited to less than the normal ten years validity period.
  • Your name isn’t changed after your most recent passport was issued. However, if your name was changed due to marriage or by court order and you can submit proper documentation reflecting on the name change, you can submit DS-82.

You can fill out Form DS-82 online, but you can’t submit it electronically. You also can’t take this form to your local USPS office, as you can apply for a US passport. What you can do is fill it out online and print out a paper copy, then mail it to the National Passport Processing Center.

California, Florida, Illinois, Minnesota, New York, and Texas residents, mail Form DS-82 to:
